        PHA leaders are required to be trained and qualified to lead PHA teams and initiatives. PHA is the cornerstone of an organization's operational risk management program and integral to occupational health and safety in process industries. PHA is performed throughout the life of a process from conception to post-decommissioning. It enables the execution of safe and compliant processes with the goal of zero harm. This four-day course is designed to train you on a wide range of qualitative PHA risk assessment techniques to comply with process safety regulations and standards, become competent and effective facilitators, become proficient with IHS software, and enable you to design and develop customized PHA solutions for your company's risk best practices.

        Concepts of risk and safety, industrial incidents and lessons learned, regulations and standards, various PHA techniques including HAZOP, What If, FMEA, and PrHA, revalidation of PHAs, introduction to management of change, human factors in industrial safety and PHA workshop using IHS software.

        Day 2 - Advanced PHA Methodology Concepts & Workshop

        Concepts of PHA team leadership, how to conduct a facilitation, role of a PHA leader, techniques for efficient and effective facilitations, how to create nodes and PHA project management, optimization of PHA team performance, what is a high-quality PHA and PHA workshop using IHS software.

        Day 3 - Software End User

        IHS software walk-through and navigation, file management, walkthrough of preformatted industry standard PHA templates, comprehensive study and practice of end-user functions, hands-on end-user functions exercises.

        Day 4 - Software Advanced User

        Introduction to IHS software template configuration engine, walk-through of a PHA template creation from scratch and hands-on exercises, comprehensive study of advanced user functions and template user interface layout design.

      • Taking the Next Step from HAZOP to LOPA/SIL

        What do you do now beyond your HAZOP or PHA study? If you find that the existing safeguards in a risk scenario are not enough to mitigate the risk to an acceptable level then what options do you have? Do you have to rely on subjective brainstorming sessions to determine additional safeguards? How confident are you of the integrity of your existing safeguards in your process? This course introduces the semi-quantitative risk analysis methodology called layer of protection analysis or LOPA that will help you to further analyze the existing safeguards from your HAZOP, in the form of layers of protection against an identified hazard in an objective and systematic way. When safety alarms, safety procedures, and physical safety devices are insufficient protection, safety instrumented systems are implemented as an additional layer of protection. The integrity of safety instrumented functions is further analyzed by SIL analysis for which LOPA is one of the intermediate steps. This course covers HAZOP, LOPA and SIL methodologies per international industry standards.

        Day 1 - PHA Methodology and Workshop

        Concepts of risk and safety, industrial incidents and lessons learned, regulations and standards, HAZOP methodology, terminology and procedures, introduction to management of change, human factors in industrial safety and HAZOP workshop using IHS software.

        Day 2 - LOPA and Target SIL Assignment

        Introduction to target SIL assignment and LOPA, ALARP, tolerable risks, risk assessment and SIL determination methodologies, concepts of layers of protection, terminology, introduction to LOPA and FTA and hands-on exercises.

        Day 3 - Introduction to LOPA

        Understanding LOPA, how does LOPA work, procedure, consequence and severity estimation, developing scenarios, initiating events and frequency estimation, IPLs, risk based decisions and workshop using IHS software.

        Day 4 - Introduction to SIL Analysis

        What is functional safety, regulations and standards, safety life cycle, implementing SIS from process conception to decommissioning, SIL verification methods and hands-on exercises.

      • Layers of Protection Analysis (LOPA)

        Layer of Protective Analysis (LOPA) is a semi-quantitative risk analysis technique. It lies in between a HAZOP and a quantitative risk assessment (QRA) in terms of its rigorousness. LOPA helps to answer the following questions:

        • What's the likelihood of undesired events / scenarios?
        • What's the risk associated with the scenarios?
        • Are there sufficient risk mitigation measures?

        This course examines the technique required to carry out a LOPA. The topics in this course include:

        • Understanding LOPA
        • When LOPA should be used
        • LOPA methodology

        Course instructors are industry experts from the fields of risk and security management and assessment, engineering principles, design and development, and projects execution.

      • Safety Integrity Level (SIL)/Safety Lifecycle

        Safety Integrity Level (SIL) is a measure of probability to fail on demand (PFD) of the Safety Instrumented System (SIS). SIL is statistical representation of the integrity of the SIS when a process demand occurs. A demand occurs whenever the process reaches the trip condition and causes the SIS to take action.

        This course covers the principles around the integrity of the safety-instrumented systems. It also teaches the users the applicability of SIL. There are in depth discussions on SIL targets as per IEC 61511 & ANSI/ISA-S84.01. SIL is used in both ANSI/ISA-S84.01 and IEC 61511 to measure the reliability of SIS. The course could include the incorporation of SIL into your facility/company risk assessment program.

        SIL determination methodologies such as Consequence Method, Risk Graph, Layered Risk Matrix, Layer of Protection Analysis (LOPA) and Fault Tree Analysis (FTA) are covered.

        Course instructors are industry experts from the fields of risk management and assessment, chemical engineering, engineering design and development, and projects execution.

        Course Objective

        Participants will learn how to conduct a SIL study the specific requirements of the guidelines laid by ANSI/ISA-S84.01 and IEC 61511, including SIL verification and validation. Also, the participants are provided hands on experience using IHS software for its SIL capabilities.

      • Quantitative Risk Analysis

        Quantitative Risk Assessment (QRA) provides an estimate of the risks posed as well as enabling risk mitigation methods to be evaluated so that risk can be reduced to acceptable levels. This training course examines the techniques required to quantify risk assessments to both evaluate and minimize risk both internally and externally to the outside community.

        The topics covered in the course include:

        • Hazard Identification
        • Consequence Analysis, including loss of containment calculation, explosion modeling, fire modeling, and dispersion modeling
        • Frequency Analysis
        • Quantifying Risk, by using techniques such as Probit Analysis

        Course Outline

        • Describes the methodology used to predict potential risks throughout the product and process design stages.
        • Learn how to effectively engage team members in the FMEA development process.
        • Learn the applicable customer requirements as outlined within standard FMEA guidelines, such as FDA 21 CRF Part 820 QSR (cGMP), ISO 14971, ISO 13485, ISO 9001, EU Directive 2000/70/EC, SAE J1739, ARP 5580, and others.

      • Root Cause Analysis (RCA)

        This course provides a step-by-step methodology to identify both the underlying root causes and the steps necessary to ensure appropriate and adequate preventive actions are taken to prevent recurrence.

        How Will You Benefit?

        • Ensure time is not wasted by team members chasing falsely identified issues and not addressing the true root cause(s).
        • Minimize the response time and failure chargeback costs associated with customer reported incidents.
        • Provide verified and validated confirmation that concerns have been fully addressed and that appropriate actions have been taken to prevent the issue and other potential similar issues from ever occurring again.

        Course Content

        • Learn the methodologies employed to ignite your company team into accurately understanding the problem, provide fast responsive interim corrective actions, reinforce customer confidence, ensure the real issues are being addressed and that cost effective permanent actions have been validated and implemented to ensure prevention of this and/or other similar concerns in the future.
        • The methodologies taught in this course are applicable to any organization experiencing either internal and or external customer related concerns.
        • Case studies will be used during the course to provide practical application experience related to the methodologies being taught.
